אני החתום/ה מטה מאשר/ת בזה את הסכמתי לשימוש בתמונות/ סרטונים שאני מופיע/ה בהם, ומסכימ/ה שהמכללה ומי מטעמה יצלמו אותי/ ישתמשו בתמונות/ סרטונים שצולמו במהלך השתתפותי בפעילויות שונות של המכללה לכל מטרה הקשורה לפעילות המכללה.
אני מסכימ/ה מרצוני החופשי כי המכללה תעשה שימוש בתמונות שלי בהתאם לצרכי המכללה באמצעי תקשורת שונים, בין היתר באתר המכללה, במדיה החברתית, טלוויזיה, אינטרנט וכן הלאה.
אני מצהיר/ה שאין לי/אני מוותר/ת על כל טענה מכל סוג בעניין זה, לרבות בנוגע לפגיעה בפרטיותי או בזכויותי מעצם הפרסום.

I, the undersigned, hereby give my consent for the use of photos\videos in which I appear. I agree that the college and its representatives may photograph me\use images or videos taken during my participation in various college activities for any college-related purposes.
I agree that the college may use my images for its media\communication needs through various social networks, including the college website, social media, television, internet, and any other venue.
I declare that I have no claims of any kind with regard to this use, including any infringement on my privacy or rights due to the publication of the images.
The publication will not include my personal details, such as residential address, phone number, or information about my financial\academic status.
